3. FILL OUT YOUR NEW PATIENT PAPERWORK ONLINE BEFORE YOUR INITIAL VISIT
Please complete our secure online forms. You will still need to bring your identification and insurance card with you to every appointment. If you choose to complete your paperwork at the time of your appointment, please note that this often increases your wait time as we have to enter all of your information into our electronic medical record and then confirm insurance benefits with your insurance company so that we can give you accurate estimates of your cost for care.
Cancellation Policy:
You will have a fee for missed procedures if not cancelled two business days prior to the appointment. Cancelled office visits without one whole business day notice will be subject to a $75 cancellation fee. Notice can be given through phone call, voicemail or explain via the contact form.Refund Policy:Approved refunds may have a 5% fee, minimum of $5.00, for handling and credit card processor charges. This applies to all payments including deposits.
